Portal-Mesenteric Suppurative Emphysematous Pylephlebitis: A Case Report

Suppurative thrombophlebitis of the portal-mesenteric venous system occurring in setting abdominal inflammatory and infectious processes is a serious condition that can lead to septic shock, bowel ischemia, hepatic abscess, death if unrecognized. Diagnosis often delayed because symptoms are aspecific pain at primary site infection may be mild. Contrast-enhanced CT scans diagnose both portal thrombosis site. Treatment include early resective surgery case appendicitis or diverticulitis, association with large-spectrum antibiotics possibly anticoagulation. A characteristic suppurative thrombophlebitis, whether splanchnic systemic, latency before effects antibiotic therapy seen. Anticoagulation administered avoid extension superior mesenteric vein. We presented critically ill 53-year-old man chronic colonic diverticulitis complicated by emphysematous only slow response antibiotics.
